Write case notes to young people
Barnardo's Plymouth Care Journey aims to improve communications between workers and young people as part of their journey app.
This example focuses on improving language when writing to young people, for example by avoiding things like acronyms.
This means case notes and an individual's 'story' can be read easily by everyone involved.
• There is guidance that the young person completes when they set up the app to outline Data protection and sharing and how the app will agreed to be used.
• When both worker and young person are available to avoid using it as a crisis management tool.
